Car will not always downshift when WOT?

Car will not always downshift when WOT? Sometimes I can go from a roll and the car works fine, other times the motor picks up load and ARF goes rich but remains locked in forth gear. It did it twice from a dig. I stalled the car and took off and it stayed in fourth. It is spurratic, and tune is accurate(WOT at 90% deactivate at 84%) . I have not Used the scanner to log TPS percent on WOT due to its rareness. Could it possibly be the TPS sensor or something in trans? All normal shifts are smooth and accurate.
Could possible be the MAX and MIN MAP setting in the TPS which are set at 65kPa and 55 kPa. I do not fully under stand what tables are affected when it is outside of these factors, to adjust properly.
