Default Spinning tail shaft bearings

I or my trans builder can not figure out why I keep spinning tail shaft bearings out. I have a built 4l60e with new output shaft and bushings. The drive shaft is a stock drive shaft hooked up to a 10 bolt. The drive shaft has been balanced and it has a new yoke. The torque arm is not mounted to the tailshaft anymore. So far I have spun out 3 bearings (one FBody tailshaft and two 2wd 4l60e tailshafts) can anyone give me any info on why this keeps happening?

Default Spun bushing

Some 2wd 4l60e output shafts come with a sleeve. Look on the plug of your slip yoke, if there is a pin hole you need the sleeve. If does not have it don't use the sleeve. Lack of lubrication to the bushing is causing your problem.
