Default Tci flex plate 399754

I have a tsp 408 with Callie's compstar crank and a 4l60e, will the 399754 work or will I need the 399753?

Correct, 399753 has the 4L60 LS bolt pattern. Just make sure the converter is built for an LS engine.

Will I have to use the spacer on that flexplate ? If so on the engine side or trans side?

The 54 was for a 4L80E style or 350/400 style snout-the FW went on the crank, then the round spacer made up the .400 diff
The 54 FW had a small spacer attached so the FW aligned with the starter
It came with longer metric bolts, but some stroker cranks had American threads-I had to hunt down correct bolts for mine
Cant remember if the 53 is the same FW w/o the snout spacer and bolts, but as said I think its a dual pattern for smaller converters
You need to meas the snout height on your converter in relation to the pads
