...at the same time? It went into REP mode. When I reset the code it just throws immediately. Has this happened to anyone else?

those are 5 volt sensor reference voltages...
I have never dealt with this specific issue, but I would take a look at all the 5 Volt sensors, MAP, (throttle assembly) - perhaps try a throttle re-learn... (assuming ETC)
Im guessing this is for a transplanted LS into another car? (check all 5 volt sensors and grounds, wiring and electrical.
The other option is to get a TEch 2 to reflash the BCM and ECM to stock calibrations as Ive read if the issue is with the BCM, would cause other issues such as REP and these codes being mentioned

I transplanted a 6.0 into my 69 Camaro and had the harness rewired. I am suspect of the harness as it was converted from DBW to DBC. Can I cut the ignition switch on and check the pink wires at every sensor to see if there is voltage? Do all of the sensors use the 5v ref.? And are they on all of the time without the engine running? I appreciate any and all help!!!
