Default Bank 1, lean, what would cause this?

So my cars been running crappy for a while...
It pops under load at low RPM... once it gets to about 2000rpm it runs fine.
I had no codes until yesterday, when I finally got one...
Bank 1 lean, it says, I cleared it, it hasn't come back yet, but the car still runs like crap.

Now I've changed the TPS, the coolant temp sensor, cleaned the throttle body, the IAC, and the mass air flow sensor....

Should I be replacing those last two sensors?
Also the car seems to have the slightest exhaust leak on the drivers side... up close to the engine. I'll take it into an exhaust shop but can this be causing it?

Any ideas?

The easiest fix could be to simply change your fuel filter.
Other problems to look for would be 02 sensors/cats.
I've had a similar problem with my truck for some time now
and have narrowed it down to a leaking intake gasket.
Depending on the scan tool you used, it should display a list of what could possibly cause your bank 1 lean condition.
Good luck

Bank 1 is the DS. The exhaust leak may be making it run lean. If fixing the leak doesn't do it replace the front o2.

+1.

It's very possible this is all caused by the exhaust leak. Make that #1 to check. After that, swap the O2s.
