Default Power Steering Pulley Came Off. Why? How??

Driving car for first time in a month. Previously driven during a street tuning session, with no problems. Decided to drive it to work last night. Ran fine, at 2nd red light heard a loud noise in engine bay. Pull off see pulley laying in street. What would cause the pulley to come off? Pump shaft "looks" ok (not sure what a New shaft looks like to compare teeth wear). Shaft spins freely. History: my rack has been leaking for some time and I filled with Lucas stop leak a while back. Fixed leak, but started leaking again last week. Would this contribute in any way? This car is rarely driven and in fact has been stationary for the most part of 3 years with change in build plans. Pump has been off block but not unhooked from rack. Pulley has never been removed or attempted to be removed in any way.
Never encountered this before and trying to figure out what all I need to replace.
Thanks guys and gals

Check the pulley for cracks. It's plastic and just a friction fit on the shaft. If your pump is binding up, maybe that could contribute to throwing the pulley, but the pulley itself isn't very durable to start with.

Pretty common on f body's. Look into the turn one stuff. They make a nice metal pulley, and some good options for pump upgrades.

Always something , Todd!!

So how do I check to see if pump is ok? Going to go ahead and replace rack and pinion, pulley obviously, and reservoir (bc pulley gauged it pretty good). Just wondering of pump is still good.....
Thanks for the replies!!!

My stock pulley was fine but I slapped a March Billet pulley on anyway...

I call it preventive care.

Honestly, the easiest way is to put a new pulley on and see what happens. The hard and messy way is to take the pump out (disconnecting it from the fluid lines) and try spinning it by hand.

Pulled everything out today. Looks like the center core part of pulley separated from the pulley itself, center of pulley is still attached to pump. Anyone seen this before?? Just curious at this point.

yes that exact thing happened to me. I run a truck pulley now. Solid steel and a bit larger diameter for reduced effort.

After seeing that, I am so glad I have a Turn One pump/pulley on mine!

Got my new Turnone Pulley and bought new reman pump yesterday. Waiting on funds to order a new rack and pinion. Mine has been leaking for a while, so figured I'd just replace every while I'm at it!
Just trying to give you time to catch up, little bro!
