Tensioner is a possibility.

Did you take off and re-install any of the other pulleys? It sounds more like one of them isnt lined up 100% or is loose.

Also, once a belt comes off it will do it easier from then on out until a new one is put on.

My belt flew off awhile back, it was the tensioner. Mine was obviously bad though, having some play.

I was having issues with throwing mine. Changed out the belt and did it again, ended up being the tensioner so I switched it out for the Katech tensioner, no problems since.

as mentioned, get the Katech tensioner and you'll never throw another belt. At WOT the stock tensioner bounces and throws the belt.

Well I actually did take off my pulleys when I did my cam swap but I know they are in there right the only other pulley will be from the tensioner. I was going to tell you guys that I can actually push the tensioner with my hand from one end to the other with out any tools. I do have to put quite of bit of force on it but I still tink it should not be that easy. I ordered the tensioner and a new belt today will have it tomorrow will let you guys know if that helps. Thanks for all the help.

I was throwing belts and it wasn't the tensioner...I had to have a belt that was 3" shorter for a 10% underdrive pulley. There are alignment arrows on the tensioner...if they aren't lined up, then there isn't enough tension on the belt to keep it from flying off.
Also a possibility that your belt has stretched over time, causing the tensioner to be too loose...causing the belt to come off at WOT.

And the spring in the factory tensioner is prone to fatigue as well, allowing a perfectly good belt to fly away.

Thunder Racing also sells a nice tensioner similar to Katech's for what it's worth.

To tension the belt, you just use a breaker bar or decent ratchet in that square hole at 10 o'clock you see in the picture above, and then tighten the bolt on the back side...sucker stays tight for the life of the belt.

The pulley is a stock unit, so when the sealed bearing wears out you can replace it with any off-the-shelf pulley and be back in business.

Another factor in throwing belts seems to be the PS pump. Mine would come off when WOT with the wheel turned, but not in a straight line. turned out the tensioner was bad later, too.

When I had a ASP pulley I had problems throwing belts until I put a goodyear gatorback on. No problems after that.

My belt just seems to jump off one rib after going WOT. Then it gets chewed to hell because of it being off a rib. I am just going to get a new tensioner and see if that helps.

mine threw a belt once, no obvious signs of wear on the tensioner although it did have a 100k on it. replaced it and it hasn't happened since.
