Wasn't there epoxy in the top hole area's?

Some do, some dont. I don't, and have dyno tested it back to back. Makes no difference.

Is it an optical illusion or was that throttle body ported too far and has holes through the casting on the left side that needs epoxy?

There are times were there is trash in the casting and its very common for it to be in those areas. I will say that for two spots that big is very rare and the one below the shaft is at a thin spot in the casting that is easy to break through. I still never have used epoxy, just TIG weld it and smooth it out.

Welding this cast aluminum that the GM supplier used is like welding water together but once you do it a few times its like anything else.
