To Degree in a cam wouldnt we need and adjustable cam sprocket?

To Degree in a cam wouldnt we need and adjustable cam sprocket? Like the title says wouldnt we need and adjustable cam gear to really make this matter. It seems to me that without it it you could only move the cam by gear teeth instead of degrees. Any input on this cause I know guys have done it on our cars.
