Default Need help guys!!!

All right guys here is what I have got going on. On the number 7 and number 2 cylinder. the intake valve lash cap is gone on my solid roller lsx 440 setup. This has caused a gap and more clearance between the rockers and the pushrod and the top of the valve. Long story short is that my Smith Brothers pushrod has now mushroomed on the ball and the rocker arm has started to wear down my valve stem. So, What do you think could cause this issue and is there anything that I can do to keep this from happening on the other intake valves? I have Harland Sharpe solid roller rocker arms. Neep help Guys .
Do you believe that the valve lash had became to great and allowed the rocker arm to smack down on the the lash cap and break it up?

Did You find pieces of the missing lash caps ? If You did , I'd say it's probably a safe bet that indeed there was too much clearance in those 2 . It's easy , if You're not really careful while adjusting the clearance , to make a mistake and not catch it even if You double check them . Personally I check and recheck and recheck them again until I'm absolutely positive that they all are adjusted to the correct specification .
