Default I have a belt dilemma help?

O.K. so I have my H/C 97 SS every time I stomp on I always run the risk of throwing the belt, or after so much of driving the car the belt gradually starts coming out of the crank pulley about two or three grooves. Now I did notice that when I took the harmonic off the hub once I saw the harmonic balancer chipped off next to the grooves do you think that could be the problem if so let me know so that I may get a knew one.

If its chipped then it may have taken a shot at one time. Check to see if it still runs true, if it wobbles it will work the belt off.
