Default Transmission Problem?

When I cruise on the highway for long periods of time (45-60 minutes) and all of the sudden decide to floor it from around 50-60mph, the rpm's will shoot up to ~3k but the car will stagnate and will not pull.

If I floor it from 50-60mph, let off for a quick second and get back on it, the car pulls fine. Around town the car shift perfectly and even getting up to speed/ aggresively rolling into the throttle. From a dig it shift/pulls perfectly as well.

Sometimes, There's a slight *clunk* from the rear when I accelerate so i think i need a new trans mount, but is that where my initial problem is coming from?

If you mean you floor the car and the rpm will only go to 3000 andthe car does not accelerate further I would suspect and engine running issue not a trans issue
