Interesting, looks like there wasn.t much holding it. Take it you haven't been in a accident to stress the floor since installing the crossmember?

After looking closer, looks like either poor penetration but the weld actually look to have split.

That looks like to be a first. maybe it scraped too many times and stressed it out-Have you contacted the maker? Yea after another look that isnt from bottoming out. not to much damage to the underside.

Jesus, it look's like it's broken at both welds. What the did you do? Catch some air an land only on that, lol?

Lol...here is the story and its nothing extravagant....it got hung going into a car wash bay....after getting it out of the bay I couldn't see any damage, but as soon as i started driving i could tell, mainly because the safety loop was barely touching the drive shaft...I had a friend weld it up for me..but shoulda contacted the manufacturer and sent it back if possible...because as you can see it wasn't welded very good the first time...
