possible causes for rev. input drum failure?

possible causes for rev. input drum failure? I lost 2nd gear and 4th was pretty weak so i dropped the trans, pulled the pump and found this.
the band is all black and pitted/worn out as well. I got a new band and a new reverse input drum is on the way...
Any ideas as to what would cause this? My guess was the servo or apply piston, but I'm not sure what a good way to test them out would be.

the servo's and piston are stock, and none of the seals were leaking. As for the accumulator pistons - none of them are cracked or anything, and they don't appear to be worn. However I did notice two of the holes in the valve body separator plate were kinda banged up and a little out of round, not a lot but noticeable.
