Default Losing reverse in my t56

I just started having problems putting it into reverse. Now its hard to get it into gear but it will go. I have to hold it in reverse or it will come out and grind. It feels like it barely goes in gear.

The tranny (LT1 T56) has about 115k miles on it, 4 puck clutch has maybe 5000 miles. I'm making 600hp or so.
What wrong?

Could be a worn out reverse gear fork or the pads on the fork. Could also be that you are not putting it all the way into gear to begin with.

I tried adjusting my shifter just incase it was out of adjustment but that did nothing.
Now it seems to be popping out of gear once and a while in 1st, 3rd too.
Any other ideas.
