Default transmission problems?

started having problems today on a long road trip... the car is a 99Z A4, mods are in sig..

i was cruising along today doing about 70. i pressed on the gas and the car revved up instead of accelerating. this happened again about every 5-10 minutes until i pulled off. i checked the tranny fluid and it was full. it only was bad when the car was warm/hot. i let it sit for about an hour and it didnt do it for another 40 miles or so. also in the past few weeks i've noticed that when i pushed the pedal a little the engine would rev up to about 3-400 rpm over what it eventually settled at. what could it be?

One of 2 things, either your torque converter is unlocking, or your 3-4 clutches are starting to go.

Question

just out of curiousity (and so i might be able to prevent this from happening again) what could be the cause of a 3-4 clutch pack going out?
