Default Need PCV tube connector part number

I'm doing an LS1 out of a '99 Camaro into a '72 Vette and I'm in need of a part number for the connector that conects two tubs (PCV) from the passanger side to one tube from the drivers side. This connector is to the passanger side of center right at the rear of the intake. I'm not sure if the connector comes by itself or not, but if possible I'm hoping to just buy it as opposed to having to buy the connector, tubes and all. It's a rubber connector with two female inlets on one side (passanger) and one inlet on the other. Any help guys?

Make your own with hose & a new PCV valve. Its cheaper & will allow you to route it how you like for a cleaner look. I'd also add in a catch can while your at it.
