Long tube headers that relocate the cats further down the stream are techinically not legal in any state, including Alaska and Hawaii.
That said, most people in states besides California have no issues with running headers as long as the a proper tune is done.

It depends. A lot of states don't even do vehicle inspections. When I lived in Indiana we had no yearly inspections. Here in Mississippi there is a yearly inspection, but just to make sure all your lights are functional.
Usually the coastal states have emmission restrictions. Can't say if North Carolina does or not, go ask a local mechanic.
