Do they do a manual inspection or just hook it to the machine to see if it passes?

Hook it up, I think.

Get a tune to clear the 'check engine' light. The emissions place will plug the car in, read no codes and pass it, its that simple.

Works only if it's an obd2 plugin vs the actual sniffer in the exhaust pipe. Check with a shop to check their inspection method, and go from there. Here in Massachusetts they do both, but depending on vehicle year it will either be a plug in at the obd2 connector, or plugin AND sniffer in the tailpipe on the rollers.

Exactly.

If it's an OBD scan only test, this is all you need to pass. The tuning just needs to be done in such a way that allows the DTCs to run (thus completing the readiness monitors), but not trip the MIL. Most tuners are familiar with this process.

If you have any kind of an actual sniff test, then your only options are to install cats or find a test location where friendship can be purchased.
