Got mine at carolina clutch for $330 plus $25 shipping. Comes with the flywheel, disk, pressure plate, throw out, pilot bearing (this is not the slave) and alignment tool.

All LS1 slaves are this way. It's just that when you buy the slave the throw out is already installed on the end of it. It comes off so you can replace the throw out without replacing the slave, but people rarely do this.
