Car grinds into 5th gear and reverse when still warming up?

Car grinds into 5th gear and reverse when still warming up? 90% of the time this only happens when the engine is still cold, within a couple minutes of starting it up. Reverse and 5th both give a sharp grind as I go into them once or twice a week, what is the problem?
