Car jerks and hard to start..

Car jerks and hard to start.. I have notice that when im driving on the freeway no specific gear or speed but the car will jerk and i will floor but still boggles. but when i release the gas and push on it again its fine and all started after i replaced the fuel filter and also same time car will start fine when its cold but after driving for a bit when i try to start it again it will just turn and turn then maybe 25 sec it will start ne ideas?

fixed the jerking part but still cant figure out why car is hard to start after running it around town? And now sometimes it wont start unless u let it sit for a while like 45 min while!!! plz help!!!!!
