Default Tranny Problem

I have about 400 horse in my 98 car. I'm unsure if it is the stock clutch but the car grinds going into 3 and it barely feels like I have 6th. It will kick out of 3 and at high rpms it won't go into 4th at all. Is this a hydraulic problem or tranny problem or both? I'm pulling the tranny this winter and will probably upgrade some pieces in the tranny and swap clutch and hydraulics but don't want to replace too much with no need. Thanks for your help.

Probably needs a rebuild with updated shift fork and to replace all the shift fork pads on the forks along with syncros and other standard replaced small parts .

Thanks,
appears that the syncros are worn and it still has an aluminum fork for 3-4 so I'll be replacing them and other parts.
