Default SDCE problem

Has anyone had problems with there SDCE tensioner setup and the blower blet walking over a tooth on the tensioner pulley itself. This has happened to me twice now. I have used a straight edge and made sure that all the pulleys line up correctly and the belt is tensioned properly. I can't see what is causing this.

Its an alignment problem, something has to be off to cause it to jump a tooth on the tensioner pulley.

Well after looking at it a little at work today it almost looks like the tensioner itself it tweaked a little. I guess I am gonna have to take it all apart and try and bend it a little.

They all look like they are on angle if you look straight down from the the top.

If the belt is moving towards the engine on the tensioner try to shim that pulley a little but (.020 at a time). In 4 years I had it jump one time, and that was because I was in a hurry and left off one of the spacers on a standoff. But even with the spacer off I put the belt back on right, made 6 passes and the belt never jumped again that day.
