Well it's possible it could be a bad/damaged opti wire harness. You could try and ohm out the wires going from the opti to the PCM. Otherwise, it's your opti. To replace the sensor, you need to remove and tear apart your current opti, might as well get a whole new OEM opti. Be sure to thread lock the rotor screws.

EDIT: That's for a '94 which I thought was what you needed because of your SN. But I see in the title of this thread that you now have a '95, which requires a vented '95-'97 opti.

ok i ordered the gm one but it didn't say anywhere on the site if it came with a cap and rotor or if it is just the optispark. where can i buy the module in case i need the module or does it come with it?

Be sure to thread lock your rotor screws, regardless of the fact that it's a brand new unit. You'll need an E4 inverted torx socket to remove the cap (you can get one at Autozone). Then you'll need a T7 (I believe) torx bit/socket to remove the rotor screws. Remove one at a time and apply thread locker to the threads and retighten the screw. A little more than snug, don't go crazy and strip it out. Put the cap back on and you should be able to enjoy a long opti-trouble-free driving.

i replaced the icm about 4 months ago with one from advanced auto parts do i need to buy an oem one? the icm kinda looks like a coil and it has a wire that connects it to the optispark right?

Yes, I would only use an OEM coil and OEM ICM.
Coil and ICM seen here:
