The module is p/n 16214681. You used to be able to get it from Summit and a supplier on eBay but I'm not seeing them anymore. Probably gonna have to grab one from a GM parts distributor.
You can get knock sensors from local auto parts stores or Rockauto.

You don't need either.
Tell us what you are trying to do and we can help.
A tuner can deal with the knock sensitivity GM used the module to address and the sensors themselves are just year dependent but even thena tuner can make say 96 sensors work on a 96 pcm.
