Default Not Again

When I bought my car the trans had been changed once and then rebuilt. I bought it 5th went out and I had it rebuilt again with a new RAM Powergrip clutch set. Didn't change the slave since it was suppose to have been done 5000 miles before I bought it. Well got it back from Aamco 5ths still out of it. So they fix 5th and then it starts grinding in the morning in 3rd and 4th. When they fixed 5th (approximantely 1000 miles on the clutch) they said it was fine. Now it dropped 2nd. Come to find out teeth are broken off second gear and the shift fork is broken. Keep in mind all this happened within 5000 miles. Now Aamco said that the clutch has about an 1/8th of an inch left on the pressure plate side. What could have caused this ? I haven't beaten on the car any at all and I don't ride the clutch. And is it possible that I use the pressure plate and just get a new clutch disk, flywheel, and slave or should I go ahead and just by a set ?
