Is this a throwout bearing shim? It came with my Spec Stage 3 clutch set up for my T56 install. Forgive my ignorance...not used to all this new LS-T56 stuff!

Yes that is a shim. I would measure the clearance before installing though.

If the bolt holes don't line up do I just use the 2 slots?

It's not going to hurt anything using the slots.
