Ram Powergrip HD slipping already?

Ram Powergrip HD slipping already? I installed a Ram Powergrip HD clutch and Ram Billet flywheel about 3-4K miles ago and suddenly last week it just starting slipping really bad. The clutch the to engage right off the floor, now if you just push the pedal in a hair its on. I did put the Ram shim behind the slave but I dont see how it could be fine for that amount of time then just start slipping. I guess Im just not seeing how I could have worn a clutch out that quick? Any suggestions?
