Default Clutch fork problem?

I recently installed a textrilla OZ700 z grip clutch in the car,has been fine for about a month of getting beat up.Last night I was driving and it sounded like the rear was locking up and felt that way something was binding not meshing right. So I got it home put in the air pulled the rear cover off everything looks fine.So I take a look in the bell housing at my clutch and I see that 3 or 4 of the clutch forks are stuck in the pushed in postion not letting go what happened here?I going to pull the tranny again oh joy and see what else I find doooh!!!!!!Any input will greatly appreciated .

Sounds like the the diaphram spring on the pressure plate broke?
Make sure your not overshimmed or overadjusted on the master if adjustable.
Yea your pulling the clutch again
