Default Serpentine belt keeps shredding

I constantly have this problem, my belt seems to be walking off the power steering pulley, I've installed a new tensioner when I installed a new belt and it didn't fix anything. Whenever I installed the belt after a few spirited drives the belt starts walking off towards the engine about a quarter inch and then after a while it will shred and come off all the pulleys, does anyone have any suggestions on what to do.

Look for one of the pulleys being out-of-plane from the
rest, sighting along the belt-plane. Look too, for any
indications that the motor is grossly jerking / stumbling
at some point in the driving envelope. which could unload
the tensioner and let the belt jump. But out-of-plane is
what would make it want to.

Some people find a need to get a better tensioner, I
myself went to a higher-lip Buick pulley of some sort,
do not remember the vehicle app now.

I just went through this on a gen 1 serpentine setup, use a 24" steel ruler, you can get one cheap from a fabric store, use it as a straight edge and check your alignment. If you mixed and matched pulleys from more than one application, that's probably your issue. I found out that camaro/roadmaster/deville, use one set of brackets, and truck/van/suv use another set, I was about 3/8" off on the a/c bracket. I know there are a bunch of different combos for LS motors.
