Sporadic low O2 B1 at WOT???

Default Sporadic low O2 B1 at WOT???

I'm looking at a friends log and I keep seeing a 451mv O2 reading at WOT on B1? Any ideas why? It's always 451 and it just pops in for 1 reading then back to normal but I see it multiple times in his log.
Car is a H/C car with headers. Any tips for tracking down what's going on there? Would a failing injector cause that reading and why is it 451 everytime? I do know that O2 on that side is brand new.

The PCM outputs to the O2s 450 mVolts, it then looks for values above and below that from what O2 outputs back to the PCM.
Either the O2 or its wires are bad, or an exhaust leak is the 1st place to look at.
