Default Stalling when going out of drive

Lately I've noticed that the car wants to stall out when I first start up the engine and move the shifter from P to R. It wasn't so bad before, but now it actually kills the engine if I don't intervene. It also does this when going into D, but not as bad.

The car is an hci with a DBW throttle and SD tune. It idles perfectly fine in any gear.

I've attached the log of this happening, the only thing I notice is the voltage drops to the mid 12s and the injectors almost max out as the RPMs drop. Any insight?

not sure then, if it tried to die in neutral i'd suggest a vacuum leak

I thought that too, but this only happens the first time after I start the car. Once I get past this, I can go from P to R all day long without as much as a hiccup.

Surely a bad iac would show up at other times too.

Try to clean inside the TB where the IAC valve slides into... also, try cleaning your MAF with brake cleaner! The injectors shouldn't be maxed out when idling though... odd

Bump, anyone else have any clue? Still dealing with this and I've tried adjusting a few things in the tune that seemed like would be of help but nothing changed.
