Default One side LTFT 25% at idle

I have one LTFT that is maxed out at idle, goes down to fairly normal values when on the gas, but jumps right back to 25 as soon as I let off. I also have some fairly old o2 sensors that are throwing a code for the o2 heaters something or other. Any chance this can all be caused from an exhaust leak? I know I have one and it's on the same side as these problems, but is that the cause for the maxed out fuel trim? I always thought a maxed out fuel trim meant a vacuum leak?

could be a failing injector or exhaust leak near the O2 sensor or at the exhaust manifolds

So an exhaust leak at the collector is probably the culprit? (I'm 99% sure that's where the leak is)

That makes sense now...b/c the o2 gets air from the outside...thinks the motor is running lean and so therefore dumps fuel...ie LTFT B1 = 25. When I hit the gas it has more exhaust flowing through so it cant suck in as much outside air...and it cant stay as warm since it's like 40 below outside (not really ), so therefore that would explain the heater code. Logic seem right?

Logic is perfect leak at the collector could definetly cause raw air in... (now if both sides were really rich, then you'd start looking for a vacuum leak)

Here is what I would do:
1) check to see if O2 is tight
2) check to make sure header is on tight
3) switch O2 sensors from side to side
4) replace O2 sensor
5) check injectors
