what would make fuel trims lock + in open loop start????

what would make fuel trims lock + in open loop start???? so the damn fuel trims were at +16 on this damn start up. what would make them be that much off? at hot idle the trims are fine, right around 0. but this is killing me on a warm start, the car runs like total ***. i would like to fix it so any tips on doing so, please let me know.
