Off idle hesitation...any ideas why?

Off idle hesitation...any ideas why?
OK, it's not really 'off idle', more like 2000 RPM to 2100 RPM, due to lean issue. Crazy part is, it's only happening on the passenger side. Look at the graph below and you'll see the passenger O2 (actually a WB sensor) goes crazy lean (18'ish AFR) at 2000RPM, while the driver's O2 is humming along minding it's own business.
I have no idea what could be causing this. Here's what I do know.
I'm guessing that somehow air is getting into the system at this time, or for some reason one or more injectors aren't firing...doesn't make sense that it would only happen at 2K RPM though.
Give me something to work with here guys

- Not O2 related. Forced car into open loop and it still occurs
- Clearly RPM/MAP dependent. Only happens around 2k RPM
- Pulled valve cover, everything is tight and looks good.
- Not related to air pump as I forced this off (and all valves) in PCM

I did the Cylinder Balance test in HPT, but didn't see a difference cylinder to cylinder. Of course this wasn't done at 2K RPM....hmm may have to try and see if lets me do it at this engine speed.
