Default Firebird rough idle and studders when accelerating hard

Hey guys this is my first post on here.

The problem is with my 1996 firebird 3800 series T5 tranny.
Just the other day the idle started to become rough. The rpm's move just slightly, but you can feel the car start to shake a little. When accelerating out of first it feels like it wants to die, but once crushing steady it runs fine. It also shudders and feels like its missing when really getting on it.

Some background information:
MAF was recently changed.
Fuel pump was changed before and you can here priming nice and loud.
Plugs and wires were changed about 40k miles ago.
I cleaned the throttle body out today with no change.
Fuel filter was changed 2 years ago.
Just ran some sea foam through the tank.
I pulled the vacuum line off the fuel pressure regulator, and didn't see any fuel in the vacuum line.
Put a new air filter in today, old one wasn't too dirty.
Exhaust is 50k miles with a magniflow cat.

The codes that shown are for the charcoal filter and the EGR, the owner before me blocked off at the intake manifold with a metal plate. Both of these codes have been on for years, and never had a problem before.

I am kind of at a loss, not really sure where to go from here.
Any help is greatly appreciated, I'm a broke college student just trying to make ends meet.
