clunk coming from adjustable poly/poly panhard rod

Default clunk coming from adjustable poly/poly panhard rod

I've heard of this problem with rod/poly and rod/rod panhards, but this has poly bushings at both ends. The clunk is coming from the chassis (passenger) side whenever I hit a bump, stop suddenly, or even just climb into the car. At one point I crawled under there and tightened the bolts and adjusting nut; that made it a little quieter for a while but the clunk is still there and is getting louder. It's been doing it ever since I installed it a few weeks ago. What am I doing wrong?

E brake cable tied up at LH side?

Did you put grease on the outside surfaces of the bushings before installing the panhard bar?

The factory mount bolts are tapered(smaller) on the nut end, so you end up with some play on that end of the bolts since the bushing sleeve isn't tapered.
