Two Tone Shift Boot (pics)
Sickspeed Shift **** Finisher (had to bore out the top of the hole a couple mm to fit the T-56 stick)
Top Gaiters Shift Boot & E-Brake Boot for an Opel Astra MK5

Shift Boot mostly installed using a 6LiterEater Designs metal shift boot retainer ring (great piece):

The dimensions for the Opel Astra MK5 shift boot and the factory 4th gen F-body shift boot were nearly identical, with the exception of the diameter of the top opening (where the shifter pokes through). Top Gaiters also makes stock replacement shift boots for our cars with the correct stick (top) diameter, but they wouldn't make them with a custom material request (black leather and platinum microsuede like the Astra MK5 boot). The quality on the leather and microsuede is quite good. Stitching is good as well, but I think Redline Goods may be a little bit better.
Still need to get a shift **** and then I'll finish the installation with the shift **** finisher piece. The shift **** finisher is used to adapt from the larger diameter shift boot opening, to the smaller diameter of the stick, and create a gap between the shift **** and the shift boot, so your fingers don't touch the boot when holding the shift ****.
Hope this helps. I paid less for the shift **** finisher, shift boot, and ebrake boot, than just the custom similarly-optioned shift boot by itself. Not sure if I can use the Opel Astra ebrake boot though, so I may go with Redline for that since theirs are the only good alternative, AFAIK.
Edit: did a loose test fit of the shift **** finisher and the shift **** outside the car (on the trans & shifter):
