It locks up fine in 3rd at part throttle or full throttle, but when It's in 4th gear it wont lock. It's not the tuning either. We've tried using the lock feature in hp tuners with no luck. Anyone ever heard of this?

Don't know, but take a look at 4th gear logs of
engine, input shaft and output shaft RPM under
various loads and be sure the converter is in
fact what's slipping (or not locking), since it
holds in 3rd.

Did this just come on for no reason, or did it
happen after some particular mod / event?

I have the same problem. It will hold in third no problem, but it slips in fourth. Mine does actually lock in fourth though, it just can't hold.
