Default belt issues on LS?

I just replaced a failed water pump & the radiator for good measure (yesterday).
I reused the belt, which looked great....well today it got kicked off, with a couple of ribs torn off, and my temps went into the red in about 20 seconds. ( boiling over)
I replaced the belt on the side of the road with my 3-year old daughter in her car seat. (Gonna keep a spare in my car now as they are under ten bucks on rockauto versus $30 @ Autozone!)

Maybe the belt was stretched to the limit of the tensioner, idk...but I can't imagine how it popped off like that.
Its got me gun shy....are there any particular issues that I should look for. I've never lost a serpentine belt before on ANY vehicle, and I've had some really sketchy belts before.

Just make sure you indeed have it routed the correct way. I replaced my belt and routed it wrong and shredded two belts.

If your routing is wrong the belt will shread on a bolt between the powersteering pump and the alternator.

Make sure your routing is like this

How are you routing it incorrectly and it is actually working at all?

Can you draw on the pic above and show it?

I have seen a few threads on thus. I think they were going from crank yo alternator. Then the power steering to idler water pump. Something like that.

Default Improper belt routing

Here is the WRONG WAY to do it...

Bad thing, its very easy to put on this way...I did not see the bolt that rubs, but there is very little traction on the crank pulley in this incorrect route setup.....there is some rubber stuck in the grooves...probably melted from slipping. The tensioner is also nearly fully extended.
