Default About time for a new clutch

Hey guys i went out to thunder valley raceway in tulsa oklahoma tonight, the temp was about 87 degrees and the DA was 3481ft.
Ok so my mods are lt headers, 3 in exhaust into a x pipe dumped before the rear axle with no mufflers, a lid and i have just got some 245 nitto 555r's. my raceweight was 3575. Okay so my best 60ft before tonight on street tires was a 2.2 and a 13.1 at 112mph. That was in much much colder weather like 55 degrees. tonight i had my nittos at 20 psi and tried pretty much dumping it at 2000, 2500, and 3000. At 2000 i just grabbed traction and it bogged, at 3000 i basically dumped it and it hooked extremely well but the rpms shot to 4000 and stayed there for a while and then i went to try and hit second and the clutch pedal was stuck and i could not get it into gear. So the clutch was sliiping badly and i only pulled a 2.1 60ft. Even though my 60ft sucked it felt like right off the line the car pulled very well, but i only ran a 13.3 at 106.
heres my slip ( also my car has 118,000 miles on it and as far as i know it is the stock clutch)

60ft 2.109
330 5.784
1/8 8.745
mph 82.81
1000 11.265
1/4 13.376
mph 106.85

I would agree that you need a new clutch.
