Yoke Needed for TR6060, 31 spline, 1350 U joint, 1.5" counterbore?

Hi all I'm looking for the part number for an OEM tr6060 yoke with 31 splines and fits a 1350 U joint?

It should have a 1.5" counterbore but I can have it modified to do so

Any OEM part number is appreciated!
