Default Excellent ABS problem checklist

Excellent post. Now I need help cuz this is driving me crazy!

So -

* Car is a 1996 Z28; has 162,400 miles on it
* Installed (17/18 in.) wheel combo back in 2004; never had ANY ABS problems
* Back in 12/2012 hit a REALLY bad pavement patch; all ABS lights turned on
* 5 days ago, I had mechanic rebuild my ABS unit; all lights were now off

Now, car will drive for a bit with no ABS lights on, HOWEVER -
* TRAC ON light will turn on/off even when pavement is smooth as glass
* As I am coming to a stop, the last 10' or so, TRAC light will turn on and
ABS kicks in a bit
* If I hit a patch of pavement that's a little TOO uneven (a.k.a sharp thump),
all thee lights (ABS INOP, ASR OFF, BRAKE) turn on and stay on until
I turn the car off and start it up again

** front ABS speed sensor/wheel hubs changed out in 2/2012 (squeaking)
** rear wheel ABS speed sensors are originals, but no error codes would come up
