A good sign is when it starts hesitating as if it's starving for fuel. I have always changed mine about every 10-15k. Speaking of which, my truck is do for one here soon.

There are other signs also, but that would be the most common one. When one clogs up or starts to clog up, you can experience sluggishness, no acceleration past maybe 40-50mph, it stutter's and dies, it won't crank at all.
Usually, whenever I get a used vehicle, a fuel filter, belt, plugs, wires, oil, etc, etc, I change as soon as possible. Most people you buy from these are the areas that aren't addressed like they should be.
