Agreed, and also:
1- Bad fuel
2- Detonation & pre- ignition
3- Too much DCR
4- Too much SCR
5- Bad crankcase ventilation
6- Bad blue printing
7- Bad geometry (PTV issues)
8- Too much boost (relative to DCR)
9- The good old piston slap (at the long run)

As long as any piston slap you may have when cold goes away when the motor warms up, it's fine and won't hurt anything. If anything, motors with mild slap won't put as much pressure on the walls under hard use.
