Default Internals & Balancing

Currently I have a 383 stroker with 4.0" crank and 6.125" rods with 3.91" pistons. I am thinking about migrating over those internals, minus the pistons, to a prepped LQ4 block. Is this possible or does the assembly need to be rebalanced for the 4.030" pistons?

Yes. Unless the new pistons/wrist pins happen to weigh very close to what the old ones did. I'm not sure how close they need to be though.

Just the rods would need to be rebalanced right? The crank should be internally balanced right?

No. The crank is balanced against whatever the pistons, rods, rings, etc weigh (reciprocating weight). Problem is if you go to larger pistons (weigh more), then weight will have to be added to the crank counterweights. Going lighter, no problem to take a little weight off the crank.
