Default 4L60E Cut-loose

I have a question on a 96 Camaro Transmission.
It was rebuilt sometime in the past. It has never shifted correctly since I have had the car. It will work ok but not great but in second gear it holds till 4000 rpm and then cuts loose. At that time one has to back off the throttle or it will go against the rev limiter. The transmission was rebuilt. One of the check ***** had worn the hole in the separator plate bigger than the check ball. The forward clutch was shot. Now it does the same thing as before. Although I am sure the transmission is shot now as it has been driven that way for some time. I still do not know where to look for the source of this issue. It was pressure checked and all looked fine.
