The throttle body connects to the right valve cover. The nipple on the right of the intake either goes to the valley cover (if ls6 style) or the pcv on the left valve cover (ls1 style). I think the small nipple is for the EVAP purge valve (if using it, or cap it). The large vacuum port on the back of the intake goes to the brake booster and that vacuum line that runs to the cab of the truck for the hvac goes on the little nipple on the back of the intake. I think!

Really comes down to your cars Requirement for Vacuum lines. Looks like a Fast Intake. .should have the same as most car intakes.
The only vacuum lines that I would think you need to be concern with are:
1) Brake booster.. located in the rear of the intank manifold, largest port.
2) HVAC - if needed for your car to operate the AC / Heater controls and doors. Use the smaller port on the rear of the Intake.
3) Only other ports on a car style intake would be the HVAP to purge the fuel tank. You can use this port for other usage. It does have a weaker vacuum signal as compared to the rear ones. else cap it if not being used. This one is located on the front Driver side of the intake.
other possible one would be if you have vacuum operated doors.
On the other side of the front of the intake is the PCV port and should draw air from a Throttle Body port, if it has one.
