Default M6 guy asking an A4 question... long

I installed a rebuilt 46LE for a guy today and would like some input on his car.(info. dont know if its signifigant, He had a rebuilt 46Le that was shipped to him without the currect tailshaft housing,it had to be changed with the current TS housing and no bellhousing was included,it was swapped from the recently removed trans.) We swapped the parts, installed the trans.Installed the correct amount of fluid and test drove the car.(Specifics of the car in a minute). On the initial test drive the car shifted normally and was ready for pick up. the owner picked it up, an hour and 1/2 later, and drove the car about 6 miles after install and on a highway strech after shifting into 3rd,OD it started hitting the rev limiter, he coasted to a stop and the car would not move forward or reverse. It made no unusall noise or etc before it quit. Any ideas on what is the most common cause of this? JFYI it did not leak any fluid or was there an obvous mechanical failure such as DS, etc. It is a STOUT moter,FMS H/C car with stall, NX kit(not used in this case) but good #'s. Thanks

Fluid was checked, it was in the cross hairs, he was letting it shift by "auto mode", no SES codes, although before the install there were 2 trans codes, which would be common place since the previous trans started slipping before it went out, we did clear those codes after the install. This car was just rolling along fine in OD and just hit the rev limiter and quit moving

He didn't ask us to flush it and he reused the same converter in the previous trans, I did recomend the flush with the above conditions before he left, but time permiting we didn't have time to perform it, but keep in mind the car only traveled about 8-10 miles total before it quit.

so the converter was used in a tranny that had issues so much that it needed to be rebuilt ? and if so and the same converter was put in a new tranny that may be the problem .sory for so many questions trying to eliminate causes...ive had my share of tranny issues as well
